I have an account with Charles Hoenisch Investigations account # 94066066601-0

3 weeks ago (approx.) I placed an order using a fax form with the account # 00029175004-0. The business is Peninsula dermatology at 1750 El Camino Real suite #207, Burlingame, Ca. 94010-9214. It was charged to my Visa account. The order form # is 618----5321.

I would appreciate notice that my order was placed and notification of any other oertanen
