Help with prescription

I want to buy the D&G Eyeglasses DD5031 with No-Line Progressive Rx Prescription Lenses and I don't know if I'm filling up the right info on the website. I've attached two images one from lenscrafter and the other from this website.

Can someone please tell me if I did it right? I don't need the SEG HT right? Since I'm getting the frame from this site. How do I know if I have Dist PD or Near PD? Thanks.

Hello and thank you for your question.

The form is filled in right. But in addition to the Rx information that you have filled in, we will also need to know your Pupil Distance and Segment Height. You can find out what your pupil distance by calling the lenscrafter's or the last place where you purchased eyeglasses. They should have that information on file.

There are two ways to get Segment Height measurement. You can place an order for D&G DD5031 frames without lenses first (here is a link http://www.opticsplanet.com/d-g-eyeglas ... d5031.html), take the frame to your local optician to measure segment height for this specific frame. And than send the frame back to us and we will complete it with progressive lenses. Or we can calculate the Segment Height ourselves using the formula that the lab has provided us with.
