Is this HDS scope recommended for 12 gauge shotgun? Also does this come with mounting hardware for a shotgun?

You can use any of the EOTech on any shotgun equipped with a weaver scope base or picatinny rail system. Unless you specifically need a nightvision compatible unit, you should choose one of the EOTech HOLOgraphic Weapon Sight 510,
