I got my order in and I noticed there is an extra piece, a small rectangular bit of plastic with an opening. Is this piece crucial? Where does it go?

It is a centering block for the lever, it’s not always necessary but handy if your specific upper/lower has different tolerances and needs the extra bit of help to stay center.

Are there any other parts needed to use this safety as designed?

Anti-walk pins and bars. These are not necessary, but recommended if you plan on longer term usage.

Great product! Well packaged and arrived on time. One thing to note though, you may have to grind away some metal behind the super safety (in your lower) to allow it to oscillate freely.
