Does this come with all the necessary hardware to mount it on the carry handle and to mount the optic on it?

Mine did. however I have read some reviews on here and on PA's website stating other wise. Mine came with the screws for both an optic and to mount to the carry handle.

The Primary Arms Prism Red Dot Sight Carry Handle Adapter is not included with the SLX 3x Microprism. This mount is specifically designed to adapt to AR carry handles and fits:
