The L states it's waist size 36-38, the XL states it's waist size 40-42. I am actually between 38 and 40. What size should I get? L or XL?

I would recommend going with the Extra-Large 5.11 Tactical Sierra Bravo Duty Belt (MPN:59505-019-XL). Since this belt is designed to be worn over your waist on top of other clothing, the XL would be most likely to fit best.

Is this the 1.75 inch or 2inch version? Also does the 1.75 inch version also come with an inner belt?

This model is a 2" belt with a 1.63" inner belt. The 2.25" model comes with a 1.75" inner belt, there is not currently a 1.75" outer belt model.
