We offer the Bianchi model 5, 6, 82, 105 & X15. The X15 only comes in Tan leather.

Since both list the same guns for fitment, and the descriptions are the same, what is the difference between the 18618 Size 3 and the 18664 Size 4?

Those are for different pistols. 18618 is for 3" barrel S&W Model 66 and similar revolvers, Item 18664 is for 1911 pistols.

Yes you can use this holster in a cross draw configuration, you would still order the left handed model.
