Dose this item fit on a 5.56/.223 barrel?

Yes, however you must match the barrel's gas journal diameter. Most pencil barrels have a .625 diameter and most other non-bull/heavy barrels feature a .750 diameter. You will need to verify or measure the gas block diameter of your specific barrel and match it to the appropriate diameter.

Yes, the .750" diameter gas block is only 1.21" at it's tallest point so either diameter gas block should fit under the Midwest Industry Combat Handguards.
