Has anyone used one of these on a rifle length gas system rifle with traditional A2 front sight block? 12.625 or 13.375" work on this build?

You would need a low profile gas block for any free float handgaurd to work.

No, you will need a low profile gas block for this to work.

Why are these handguards not for use with gas piston systems? Is it a clearance issue with the gas block? Low profile gas block won’t fit?

Will this fit over a KAK Industry flash can?

Providing your can has a smaller diameter than 1.3 in then it will work.

Will this fit Daniel Defense DDM4 v5?

Yes it will work for the DDM4 but you will need to remove the proprietary DD barrel nut and gas tube/block and replace the nut with the one that comes with the handguard

Yes it will work with any low pro gas block. However, it will not work with the SA piston gas systems as they have an OD of 1.57”

Would this work on S&W sport 308

This will not work on any 308. This handguard will only work on the AR15 platform.

I am lookin to run the 15in on my S&M MP Sport II, will I run into any issues or need additional parts?

Chances are, your S&W MP sport 2 has an A2 front sight/gas port which will not fit under or be compatible with this handguard. You can either grind off the front sight down to the gas port to make it fit under the rail or replace the A2 gas port with a low pro gas block. Then it will work.
