Are the mount and swivel one piece? Or can you disconnect the swivel from the mount?

The swivel can be released from the mount by use of the push button.

Will this work on a Galil Ace picatinny rail and fit through the hole that is integral to the forearm cover grip?

This product is compatible with any picatinny rail, not sure what is meant by "hole that is integral to the forearm cover grip".

Do these come 2 to a package? Will it fit the new Ruger PC9 Carbine?

They are sold individually. These are designed to mount to picatinny rail, the Ruger PC9 has traditional sling swivel studs compatible with sling swivels such as the following:
