Does this include bolt and carrier?

No it does not include the Bolt Carrier Group or Charging Handle.

Does the 15” version come with the forward assist or shell deflector only? There doesnt seem to be a place to choose that option.

The uppers listed on this page have both the forward assist and shell deflector. If you are looking for the model with just the shell deflector, then you will want one of these instead:
