Purchased here from Optics Planet and it arrived today. I had no problem installing it and also tested the functionality by fulling loading the upgraded mag tube (holds 8 now!) and cycling them back out to unload. My gun was purchased new June 2020. It took a very long time finding one that had the proper threading. While Choate also makes one for the (CZ) 612, the price was better from Optics Planet due to the free shipping.
