Came loose after 15 rounds - JE Machine Tech AR-15 Muzzle Brake

The muzzle brake is a good quality, tough. The crush washer is flimsy, I torqued it correctly, and it came loose. Now I need to get a crush washer , is gonna be steel for sure. I wish they included a steel crush washer.
