Yes, the mount will include both smooth and picatinny tops.

Yes a total of 4 ring top halves. I use the rear rail top for a bubble type level, smooth in front. Huge adjustment range for eye relief. Mounting Burris 5x25 XLR II on an Armalite AR-10A in 6.5mm Creedmoor.
