What mount do I need to mount this on my AR15 picatinney rail?

The Bushnell 4-12x40 Rifle Scope Matte Rifle Scope requires a 1" scope mount. Since you are mounting it to an AR15, I would recommend a high mount like the Primary Arms AR15 Basic Scope Mount, found by using the link below:

Bushnell describes the warranty as:

BUSHNELL IRONCLAD WARRANTY
The Ironclad Warranty is a full lifetime warranty that covers the defined lifetime of a product. Lifetimes can range from1 to 30 years. A Product’s lifetime can be found below and/or on the Bushnell webpage specific to this Product. If this product isn’t working properly due to a covered defect, we will, at our option, either repair or replace the product and ship it back to you at no charge. This warranty is fully transferable and does not require a receipt, warranty card, or product registration*.
