It is the NexYZ 3-Axis Universal Smartphone Adapter and it also includes the TrailSeeker Fluid Pan Tripod.

Yes, it is the NexYZ 3-Axis adapter. I have not had the chance to us the tripod yet, but to look at it, it appears to be well built and easy to use. I believe this pod is the same or similar to the Trailblazer Fluid Pan tripod that is sold separately here at Optics Planet
