Will this work with the Magpul M-LOK MOE forend? This forend extends past the extension tube nut by at least 1/2”.

The GG&G Remington 870 Sling and Flashlight Mount Combo will not work in conjunction with the Magpul MOE M-LOK Remington 870 Forend.

Which side is rail on for the "left handed" model?

The Left-Handed GG&G Remington 870 Sling and Flashlight Mount Combo has the sling attachment point and flashlight rail on the right side of the shotgun.

The GG&G Remington 870 Sling and Flashlight Mount Combo model GGG-1617 is the right handed model and model GGG-1617L is left.

I see that there are 2 models avail. If I want the sling to attach on the right side of the gun, which one should I order ?

The model number that ends in L is for the left side. For the Right side, you would select the first option.
