Does this come with the comfort plus system, or will that have to be bought separate?

It does not include the Comfort PLUS pads and they will need to be purchased separately.

My kids got me a Sightmark Ghost Hunter 1X24 Night Vision goggles. How would you recommend I attach them to this helmet?

If I have a 23 1/2 " head, should I buy a medium-large or large-extra large size helmet? Do sizes account for internal padding?

According to the manufacturer’s FAQ: if you are on the cusp between two sizes you should go UP a size.

What hat size is a large vs. extra large helmet?

The manufacturer does not provide these sizes in "hat" size measurement, however, the medium size has a circumference of 20 1/4"- 23 1/2" and the Large has a circumference of 23 1/3" - 24 3/4".
