i am a 10.1/2 but i have flat feet and were orthotic arch supports . depending on the shoe i get an 11 or a 12. which size should i get that will fit

That is hard to answer since boots fit differently between models and brands but if you normally get an 11, I would get an 11 in these Kenetrek Mountain Extreme 400 Boots.

I recommend contacting the manufacturer in regards to having the Kenetrek Mountain Extreme 400 Boots re-soled.

Does the boot run small - I wear a 10 1/2 in foot wear normally- would I need the same size or should I get 11

You can order the size 10.5 and it should fit well.
