I measured 11 1/2 foot, should I go up to 12 or 12 1/2 on the boot?

from other measuring charts it seems that I would need a EE in a size 9. If I order this boot in a "Wide" size 9 would that be equivalent to the EE?

If I order 1000gram insulated boots do I need to go with the next size up?

The fit is true to size, but if you are going to be wearing thick wool socks or thick liners then going up a size would not be a bad idea.
