Does this come with all the lenses or just the prism tr22? For the 150 dollar one

Each model has different included accessories, you will want to choose a model option that states it has replacement lenses included.

Is the red laser visible with the glasses

A red laser would be visible with the glasses if you can't see it well with shades on put the white glasses on them to help see it better.

How is proper fit calculated? I assume protocol applies to all Oakley ballistic frames? I will also require prescription lenses.

Oakley SI will be able to provide you more information regarding sizing and fit of their frames. Unfortunately, we do not offer prescription lenses for these sunglasses. You will have to acquire them from vendor that specializes in prescription lenses for Oakley frames.
