Do you also need to purchase the Streamlight Sidewinder Stalk Arc Rail Clip 14304 separately to attach your sidewinder stalk to this?

Yes, the Streamlight Sidewinder Stalk ARC Rail Clip must be used in conjunction with the Sidewinder Stalk Helmet Light in order to mount to this Streamlight Sidewinder Hook and Loop Adapter Plate.

does this have the hook on it? Also, does it have the screw to attach the light?

The Streamlight Sidewinder Hook and Loop Adapter Plate features a Velcro backing and fits the Sidewinder Stalk Helmet Light by sliding the light in the rails of the plate and using the thumb screw on the light to secure it.
