Do the small holes go on bottom or top?

The ports on the TacFire Crown Muzzle Brake will be oriented towards the top in order to mitigate muzzle rise.

Will this work with a Radical Firearms 16" 300 Blackout Complete Upper with 12" FCR with a 5/8x24 muzzle thread? My concern is the bore diameter.

The .308 versions of the TacFire Crown Muzzle Brake (MPN:MZ1024-3B and MPN:MZ1024-3SS) are compatible with your upper and 300 Blackout. 300 Blackout shares the same bullet diameter as .308.
