Does this attach only to stock milspec bolt releases?

Yes, the Tiger Rock AR-15 Extended Bolt Catch Release Lever is designed to attach to Mil-spec AR15 bolt release's and extend into the trigger guard, allowing for ambidextrous manipulation.
