Can you use this carry handle without the BlastAR Handguard?

No it's meant to be used in tandem with the handguard.

What is the total length of the Vism BlastAR Handle Mount?

The Vism BlastAR Carry Handle Mount will fit onto any 14.5" length of picatinny rail. It features a .5" picatinny riser that is 5.25" long. The carry handle is designed to be mounted around the front sight post in a forward position over the handguard and the forward portion of the upper receiver.
