How to insert the konus barlow between my dslr and my telescope? - Konus 2x Barlow Lens 31.8mm with Photo Adapter for Konus Telescopes

I purchased the Konus 2X Barlow to insert between my DSLR camera and my telescope so that I could increase the effective focal length of my scope. The only way I can figure out how to make the connection requires a two inch cylindrical sleeve having male threads on the outside. As of yet I haven't been able to find where to by this sleeve so I can't use the barlow as I had hoped.
Pros: T-adapter built into a 2x barlow
Cons: Can't figure out how to connect barlow between camera and telescope
