So all I need is the tail hook for this to make it a braces for the scorpion

Yes. This Gear Head Works Scorpion Tailhook Mounting Kit will fit the CZ Scorpion EVO, all you will need is the Tailhook Mod 1 to complete this package for your Scorpion.

Is this a polymer brace? Or is it constructed of aluminum like the separately sold tail hook?

This mount is made out of a durable polymer. It is not the brace but will fit the Tailhook Mod 1 to the CZ Scorpion EVO 3.
