Yes. Just put it on my P365lx yesterday. Fits nicely and comes with the proper length mounting screws.

Yes. The 365XL uses the RMSc footprint. The Holosun 407/507k has the RMSc footprint. No mods required.

Yes it will, but you have to take the slide all the way apart to mount it or any Opti.
