The Magpul Industries M-LOK Hand Stop Kit attaches to MLOK rails. So it will work on a Smith and Wesson M&P AR Pistol.

Can you just use the handstop? Or, does it need to sit on the handguard rail that comes with it?

These are designed to fit properly on any MLOK rail and any other dimensional questions should be referred to the manufacture.

Can't this be used with a 5.5" hanguard, that only has two mlock slots? Maybe if one of the pieces have to be left off?

The length on the three panels is approximately 4.75 inches. Each panel covers approximately 40 mm of space or a little over 1 full M-lok slot. You can also use each piece individually.
