Radical Firearms does not specifically state if the Radical Firearms 16 in. 5.56 SOCOM Upper Assembly barrel is lined. However, they do state that the barrel is Melonite Coated.

Some of these do came with the BCG and changing handle, some do not. If you use the Additional Features filter, you can find the models with and without those parts.

I have an m&p AR15 should only have to switch out the upper? nothing else has to be down right?

Yes, you would only need to swap the uppers.

Does the flash hider come welded or can I take it off and put a different muzzle on?

No, the muzzle device should not be pinned and welded. You should be able to replace the muzzle device.
