The "nub" side should be facing towards the muzzle thread shoulder. Here are the install instructions:

To install your slim muzzle device, follow the instructions below:

Install the crush washer onto the barrel with the thinner "nub" side facing the muzzle threads shoulder.
Lightly thread on the muzzle until it just starts exerting pressure on the crush washer with your hands.
Use the flats to the muzzle device to slowly turn the muzzle device until it is timed. These custom crush washers only have about 230° of crush, so once one side is aligned - STOP!

What is the distance from the rear of the brake to the first port?

The first port starts .720" from the rear face of the muzzle brake.
