Could this handle a spike tactical 37mm flat launcher

No. The NcSTAR AR-15 Picatinny Handguard Rail is tapered to match the handguard taper of the standard GI carbine and M4 handguards, it will not fit your 37mm flat launcher.

If your M&P 15 uses the standard polymer handguard/forend, then the NcSTAR AR-15 Picatinny Handguard Rail will fit and function as intended.

Was this designed to fit standard A2 handguards without modification? Or is it a regular section of rail that will fit only with some cutting?

This rail is designed to fit into the circular holes in the mil-spec handguard.

This rail is for neither MLOK or Keymod. The rail mounts to M4 style handgurards.
